Chronic disease management is a core role of general practice and GPs.
From 1 July 2025, there will be a new framework for Medicare chronic disease management (CDM) items. Medicare items for chronic disease management will be simplified to a single item for plan preparation and second item for plan review. Transition arrangements will be in place to ensure continuity of care for patients currently under the existing CDM items.
The RACGP is committed to supporting GPs, practices, and patients through this transition.
